Changing Trends in Management of Acute Cholecystitis in Korea



Abstract
The subject of acute cholecystitis already has been comprehensively reviewed at various times, yet the aspect of it's surgical management remains controversial. The aim of this study was to focus on the recent trends by demonstrating the
principal
differences between 2 series of patients operated on during 10 years, and authors observed on the difference by timing of surgery(early vs delayed) during same period.
Forty two consecutive operations for acute cholecystitis were performed from 1982 to 1986 (period I) and were compared to 62 cases operated from 1987 to 1991 (period II).
@ES Several trends were observed in period II.
@EN 1) The frequency of acute cholecystitis increased in benign gallbladder disease.
2) The population was significantly older with a higher proportion of males.
3) There was a significant increase in common bile duct stones, gangrenous changes in the gallbladder and positive bile culture.
4) There was a significant increase in patients with intercurrent diseases and without previous history of biliary symptoms.
5) There was a significant increase in patients operated within 48 hours of admission.
6) There was a significant increase in subcostal incision and exploration of CBD at operative procedure.
In particular, a decrease in rate of postoperative complication, technical difficulty at operation recorded and transfusion given was observed at early surgery group compared with delayed surgery group.
